The Attendance Certification Form is to be completed by the attorney after participation in this activity. Keep one copy for your files and return one copy to ACWA by mail at 910 K Street, Suite 100, Sacramento, CA 95814 or by email at events@acwa.com. This record of attendance will be kept by ACWA for a period of four years and, if ACWA is audited by the California State Bar, will be submitted as official documentation of your participation.
